Why AI Automation Projects Stall (And What to Do About It)

We've worked on dozens of AI automation projects across financial services, healthcare, media, and government. The ones that stall tend to hit the same five walls, and most of them have nothing to do with the AI itself. They're about picking the wrong process, underestimating data problems, confusing a working demo with a production system, involving compliance too late, or neglecting adoption entirely.

Here's what each of those looks like up close, and how to navigate them.

Picking the right problem

A client almost paid us six figures to build an invoice reader that would have saved their team 10 minutes a day.

They wanted to "automate invoice processing." Reasonable brief. But when we sat with the people actually doing the work, the bottleneck wasn't reading invoices. It was chasing approvals across three departments, each with their own sign-off rules. Extraction took 10 minutes. Approval routing took 3 days.

Before writing any code, sit with the team and observe the full workflow as it happens. Time every step. The real constraint is almost never where the initial scoping suggests it will be.

Data quality is always a surprise

Once you've identified the right problem, you need data to work with. This is where even well-scoped projects run into friction.

On a fund intelligence project, we had access to 8 years of financial documents. Thousands of prospectuses, SAIs, annual reports. Looked like a strong training set until we dug in. Every fund manager reported differently, and even within the same firms the formatting had shifted multiple times over the years. The OCR on older scans was unreliable, and roughly 6% of the files were duplicates under slightly different names.

Data accumulates in messy ways over time, and there's rarely a reason to audit it until a project like this surfaces the gaps.

Before writing any code, assess the full dataset. Pull records from different time periods, different sources, different formats. If there are quality issues in a representative sample, they exist at scale. Better to surface that early than to build a system that assumes clean inputs.

The gap between prototype and production

The prototype phase tends to feel like progress. The demo works. The test cases pass. Stakeholders are engaged. Then you connect it to real data at real volume and accuracy drops from 95% to 72%.

Where does the gap come from?

"Edge" cases that aren't edge cases. Test data is clean by definition. Production data has exceptions nobody thought to mention because they handle them reflexively. On the fund document project, the system needed to parse 14 different fee table formats. Our initial prototype handled 3. The other 11 weren't edge cases, they were most of the data.

Scale. Processing 100 documents is a different engineering problem than processing 10,000. Memory management, API rate limits, timeout handling, retry logic. None of it matters in a demo environment.

Integration. The prototype ran on a laptop with a CSV input. Production means authenticated connections to document management systems, databases, downstream workflows, and orchestration across all of them.

The way to avoid this: build production scaffolding from day one. Real data connections, real error handling, real logging. A prototype built on actual integrations can evolve into the production system. One built on synthetic data in a notebook will always be discarded.

Getting compliance involved early

In regulated industries, there's a recurring pattern where legal and compliance review the system after it's mostly built. Their questions are entirely reasonable: where is data being transmitted, which models have access to PII, how are decisions audited, what happens when the model produces an incorrect output.

These are answerable questions if the system was designed with them in mind. Retrofitting audit trails, access controls, and human approval gates into an existing architecture costs 3-4x what it costs to include them from the start. It's less expensive and faster to build it right the first time.

Adoption is its own project

Something that caught us off guard early in our practice: you can build a system that performs well, deploy it successfully, and still find adoption sitting at 15%. The team continues working the way they always have.

The reasons vary: people don't trust the output, they weren't trained properly, or they see the tool as a threat to their role.

What we've found consistently works: deploy to one team first. Choose the team dealing with the most acute version of the manual process. Make adoption voluntary. Address every point of friction they surface, quickly. Let it spread through the organization because people saw it working, not because someone told them to use it.

From day one, show the internals. Let people see how the AI arrived at its recommendation, and give them the ability to approve, revise, or reject it. When users feel in control of the system rather than subject to it, adoption takes care of itself. On one project, adding that single layer of visibility moved adoption from 15% to 90% within three weeks.

What connects all of this

The common thread across every stalled project we've encountered: the team approached it as a technology initiative. Build the model, deploy the model, move on. The model accounts for roughly 20% of the effort.

The remaining 80% is understanding the actual process, dealing with messy data, building for compliance from the start, and getting people to change how they work. Operational problems, all of them.
